The Coverage Manual is a comprehensive document that sets forth the rights of the plan's participants & beneficiaries, as well as guiding the university in making decisions & executing our responsibilities. 

The ISU SSHIP Plan has three levels of coverage. Where you receive your care affects how much you will spend on our-of-pocket costs, depending on the level of coverage.
