Treatment provided in an office visit, or outpatient setting; intensive outpatient setting; or outpatient partial hospitalization setting Services and supplies associated with certain disorders related to early childhood, such as academic underachievement disorder
Individual, group, or family therapy provided in a clinically managed low intensity residential treatment setting, also known as supervised livingServices and supplies associated with communication disorders, such as stuttering and stammering
Treatment, including room and board, provided in a clinically managed medium or high intensity residential treatment settingServices and supplies associated with impulse control disorders
